Adults with SPMI, other disabilities, and the elderly. |

Needs assessment, goal planning, skills training, case coordination, referrals to other agencies, organization of in-house activities and health and wellness speakers, data tracking/HUD reporting. Position is based on-site at a St. Louis Park high-rise, requiring the ability to plan and work independently. |

Standard hours are Monday – Friday, 8:00 am to 4:30 pm |

|
Bachelor's degree in human services field + minimum of 1 year's experience working with people with mental illness
1 year's experience in housing or housing support preferred
Must have car |
