Summary of the current status of Vail Offices, programs and services.

  • Facilities remain closed.

Targeted Case Management

  • Case Managers continue to focus on maintaining stability in the community for all individuals and connect with individuals virtually/over the phone and see people face to face for critical safety concerns.
  • The Case Management program is currently accepting new enrollments.

Vail Care (Behavioral Health Home)

  • Navigators and team members will continue to focus on maintaining stability in the community for individuals and connect with individuals virtually or in person based on intensity of need.
  • Vail Care is currently accepting referrals.

Vail House

  • Vail House continues to meet the needs of our residents within the home. Pausing community activities, time away from the House, and following physical distancing recommendations.
  • We are limiting visitors and vendors to the House.

Vail Connect (in partnership with North Memorial Health)

  • Navigators will continue to focus on maintaining stability in the community for individuals referred by North Memorial Health.
  • Navigators will connect with individuals virtually or in person based on intensity of need.

Mental Health Connect (in partnership with Bethlehem Lutheran Church)

  • Navigator is continuing to focus on providing referral services for individuals referred through the faith-based community on a part-time basis.
  • Navigators will connect with individuals virtually or in person based on intensity of need.

Results Focused Model (in partnership with Health Partners)

  • Navigator is continuing to focus on providing intensive care for individuals referred via the Health Partners Care Coordination team.
  • Navigator is currently working from home and will continue with individuals virtually or in person based on intensity of need.

Housing Support Services in the community and at Louisiana Court

  • Maintaining housing during this time of uncertainty is critical, therefore, housing staff will continue to meet with individuals and property managers as needed virtually and in the community.

ROSS Coordinator at DOW

  • The ROSS Coordinator is onsite 2 days a week.
  • The Coordinator will continue to accept referrals from the HRA and will continue to focus on maintaining stability for residents.

ROSS Coordinator at Hamilton House

  • The ROSS Coordinator is temporarily working from home with access to the building restricted.
  • The Coordinator will continue to accept referrals from the HRA and will continue to focus on maintaining stability for residents.

Vail in the Park Apartments

  • Vail in the Park currently full with the eight residents going about their daily life.
  • The property management company (Marathon Management) in coordination with Vail Place is limiting visitors and vendors to the building.
