Clubhouse Community Support Services

The Clubhouse Model centers on community, relationship building, and opportunities for you on your recovery journey. These opportunities help you find friendships, employment, education, housing, health and wellness, and a broader connection to the community. See our Clubhouse Page for more information. We operate out of two Clubhouse locations:

Case Management Services

A case manager works with you to connect you with the services you need and want. The goal is to achieve recovery, health and independence. Our case managers meet with you in your home or elsewhere in the community at least once a month or more frequently if necessary.

Vail Care Services

Vail Care is a comprehensive service provided by a team of individuals, a nurse and others who can help you with all aspects of your life. The Vail Care team can assist you in securing mental health and medical services and finding social and community supports. Our staff work with you in your home as well as the community to create a Health Action Plan, as well as provide coaching and support to help you meet your health goals.

Vail Connect Services

Our Care Navigators can help you, your family or your other care providers obtain mental and/or physical health assessments, treatment, support, resources and education. It is a mobile service intended to be short-term, typically up to three months. Care Navigators can meet you in your home or elsewhere in the community to arrange services and supports. Care Navigation Services are limited to people connected to North Memorial Health and affiliated clinics.

Vail House Services

If you are a homeless adult who lives with mental health and chemical dependency needs, Vail House can help you with transitional housing in an environment that is safe, secure, and nurturing.  You will have access to recovery-oriented support services that promote independent living.
